Request to Repeat a Course a Third and Final Time

This form is required for students attempting a third and final repeat course enrollment. The LMU Bulletin policy on course repeats states “students may repeat a non-repeatable LMU course once, including withdrawals. The third and final time a course is taken requires approval from the Dean.”
