Below is a list of LMU's most commonly used academic forms, with instructions for using them. Each of these forms involves slightly different steps for completion, but all end with submitting them to the BCLA advising center via email for final approval.

When To Use:
If a requirement is not plugging into your Degree Works, please meet with your Department Chair. They can submit a Degree Audit Adjustment form to substitute major and minor requirement courses on your Degree Works Report.
How To Use:
- All Degree Audit Adjustment Forms must be initiated by a chair or program director.
- If the substituted course is a major requirement, it must be initiated by your department chair. If the adjustment pertains to a flag or core requirement, please meet with a professional advisor in the BCLA Advising Center.

You'll use the above form to petition for an exception to a university policy.
How to Use:

DOWNLOAD INDEPENDENT OR TUTORIAL STUDIES FORM
When to Use:
You’ll fill out this form prior to course registration to sign up for an independent study course with a faculty member. The form must be signed and completed by the time you register for classes.
How to Use:
- Fill this out in collaboration with the faculty member with whom you will be taking the independent study. That faculty member will need to sign the form.
- Submit the completed form for processing to BCLAAdvising@lmu.edu. Advising staff will email or call you only if there are follow up questions.
- Note: If the independent study course is housed in a department outside of BCLA, you will need a signature from that dean's office as well.

When to Use:
Use the late add petition to add or withdraw from a course after registration ends.
How to Use:
- The approval signatures on this form vary by issue, but are named clearly on the form.

You'll use this form when trying to enroll in two classes that are scheduled during overlapping/conflicting times.

You'll use this form to receive LMU course credit for courses taken at other institutions.
How to Use:
- Make sure your course will qualify for transfer credit BEFORE you sign up for the class.
- First, find your class in LMU's list of Transfer Articulation Agreements, a comprehensive list of courses at regional institutions that do qualify for transfer credit. View it here.
- Then, fill this form out and seek approval signatures before you register for the course. Please note: Courses from an accredited and LMU-recognized U.S. institution with a grade of C or higher are acceptable for transfer. LMU credit will be given only after the receipt of an official transcript from the school attended at the request of the student. A combined maximum of 60 semester hours may be transferred from community colleges. Only approved courses on this form are eligible for transfer.
If you cannot find your course on LMU's list of Transfer Articulation Agreements, please complete the Transfer Course Review Form with the course information. The request will be reviewed for potential articulation and credits by the Registrar’s Office.
- As indicated on the form, courses that you will hope will apply toward major or minor requirements will require signature of the chair or program director. Core and elective courses only require dean's office signature.
- Make sure your course will qualify for transfer credit BEFORE you sign up for the class.

TRANSFER COURSE REVIEW
When to Use:
You’ll use this form to apply coursework from study abroad programs toward your major, minor, and/or core requirements.
How to Use:
- This form is part of the study abroad application process, administered by the LMU Study Abroad office. Check off the Study Abroad box at the top of the form. Study Abroad staff can help you address additional questions about the signature/approval process for this form.

You'll use the above form when you want to adjust the number of units a course is worth past the deadline.
