Forms Explained

Frequently Used Forms

Below is a list of LMU's most commonly used academic forms, with instructions for using them. If you do not have access to a printer, come pick up a printed copy in the BCLA dean's office (University Hall 4600). Each of these forms involves slightly different steps for completion, but all end with dropping off at the BCLA dean's office for dean's signature.

Think you need something else? The Office of the Registrar maintains a complete list of student forms here.
